Patronage nurses of the Community Health Center of the Primorje-Gorski Kotar County participated in the Urban Health Centers Europe 2.0 project. The project was realized in collaboration with the Faculty of Medicine. It started in January 2014 and lasted for three years. Through project was promoted innovative and integrated health and social care, early detection of infirmity, polypharmacy management and fall prevention with the aim of active and healthy aging.
The model was simultaneously implemented in Rotterdam, Manchester, Pallina, Valencia and Rijeka. The patronage nurses within the project were tasked with collecting data, filling in questionnaires, categorizing users based on assessment and interventions, or including older people in the project.
The questionnaire examined their needs and directed them to the activities they could apply. One group was consisted of people who through their tablets regularly recorded taking the drugs, the second group was consisted of people involved in a workout program to prevent falls and improve their general condition, and the third group was consisted of people in need of companionship and communication.
The main aim of the project is to enable the elderly to actively and healthyly age and extend their life expectancy.
The patronage nurses were recognized as unique and helpful contributors within the project.
