Social Support Program in Mental Health and Social Primary and Primary Care

Abstract

The networking is indispensable and a challenge for the Municipal Social Workers who work at the Social Services, the Sanitary Social Workers and the Social Workers Specialized in Mental Health.

The coordination, support and exchange between professionals is an enrichment for the patient, and it guarantees him integral attention. These three things constitute for the patient a continuous attention process in order to treat the complexity of the cases of the Severe Mental Retardation, or patients with similar cases.

The Social Support Program in Mental Health and Social Primary and Primary Care, describes a working methodology of proactive work and proposes to work from the bottom of the holistic view of the patient, using as tools the treatment and the prevention of social situations that can be derived or coexist with mental illnesses.

The main aim of the program is to promote a cultural change in our services in order to optimize social actions considering the Social Diagnostic and the social service portfolios.

On the other hand, it considers that good psycho-social management, in real time and in parallel development and evolution of the medical treatment, provides efficiency to our sanitary system social actions.

For this reason, the pilot trial of programme, was launched for a year in some territories of Vallés Oriental (Barcelona, Catalonia). During this year of operation, it obtained good results, holding 27 meetings for 111 cases related to mental health. The municipalities has created an effective and efficient tool in order to ensure the integral attention centred in the person giving answer to the needs of the Sanitary Social Diagnostic.

This programme proposes a working method based on creating spaces for staff training, for the purpose of finding a way to achieve a COMMON BOARDING PLAN FOR MENTAL HEALTH PROBLEMS, specific to each district health center.

We can draw from the professional's valuations:

- The Programme involves the care agents working together with respect knowing the sector's limitations.

- The Programme takes special note of tools for technicians since it makes profitable time and makes the actions more efficient. 

Finally, the programme offers a conceptual frame and a model of coordination between Mental Health and Primary Care Attention.
