The contribution of social factors in the genesis and evolution of the health problems of people as well as the importance of social and family support as a protective factor for health have been extensively studied in various scientific forums.

This evidence highlights the need for addressing social problems of health and social workers trained in the sanitario- -widely field are present in each of the assistive devices that provide health care to the population. It should work on problems of social functioning health-altering, and those health problems that impair social functioning of people and their environment.

The Health Consortium of Barcelona, in line with the Program for Prevention and attention to the chronicity of the Generalitat de Catalunya, has a group of social workers of different

levels of care (hospitals, primary health care, health centers, home care teams , mental health, care homes, and drug dependence) of the city of Barcelona, they meet with the aim of promoting and defending the importance of social factors in the care of the chronically ill and clarify and strengthen its role.

After two years of work, this group reflected on their professional role in relation to chronicity by reading and discussion of articles and documents, as well as analysis of the experience of the members of each device, creating a consensus document which they identify each and every one of the services offered to citizens.

They conclude that in general, all health social workers of any assistance device offered to the population common services (proactive detection, medical social diagnosis, plan of work, social treatment, group care, coordination and networking, community health and promotion volunteering, participation in welfare committees and research and teaching).

And specifically, each health worker, offering a differentiated according to their level of care (hospital, primary care, geriatric, ...), being widely detailed in the portfolio of services developed by this group.

Overall, this work offers a vision care continuum, a line of Cross health care by social work, which appears or takes over care in any health care device providing continuous support to the patient and family, regardless of the path the chronically ill do.

Accompaniment, when required, should be coordinated and connect with other community services that help complete and comprehensive care to chronic patient and family reality.

The results of this work have been:

- The creation of a portfolio of health consensual social work in the city of Barcelona to the attention of chronicity.

- The commissioning relationship between different levels of care professionals and providers (public and private) increasing trust and allowing the establishment of new synergies.

- Strengthening the role develops social work in health chronicity in our city.

- The empowerment of the health worker in the management of your organization, promoting their participation in new care programs chronicity.

- Improving the continuum of care for the person and their environment.

possible areas for improvement and research to advance the social care of some populations are displayed; children with chronic disease, people with addictions and family violence situations.

In short, it provides, the result of research, reflection and consensus, the first of a series of future instruments that allow guide citizens, institutions, managers and professionals in the important task is to develop the health worker in the city of Barcelona.
